Application forms may be obtained by downloading them from the NIU Graduate School. When completing the form, list the major specialization code on page 3 as #263 to specify the Specialization in Human Anatomical Sciences.

will be sent a packet of application materials to be completed and sent
to the Graduate School. They can also be contacted regarding any general
questions about graduate study here at NIU. For your application to be
considered, we must receive all of the following material: completed application
form, $30 application fee, transcripts, letters of recommendation, and
Graduate Record Exam (GRE) scores for the general test.
